Required information
A notice should include:
- Identification of the copyrighted work claimed to have been infringed.
- The exact URL and enough information to locate the material.
- Your name, address, telephone number and email address.
- A good-faith statement that the disputed use is not authorized.
- A statement, under penalty of perjury, that the notice is accurate and you are authorized to act.
- Your physical or electronic signature.
Submitting a notice

Counter-notices
If material was removed by mistake or misidentification, the affected party may submit a counter-notice containing the information required by applicable law.
